Immunization Records
In accordance with Kentucky immunization law (KRS: 158:035), no child shall be eligible for entrance into The Lexington School without first presenting an Immunization Certificate from a duly licensed medical practitioner or facility in Kentucky stating all immunizations are current (out of state and international families should contact Kristin Beers at kbeers@thelexingtonschool.org for more information).

Effective July 1, 2018 all  students in kindergarten through twelfth grade must show proof of having received two doses of Hepatitis A vaccine to attend school.
 
Additional Requirements for Kindergarten or Montessori 3:

Verification of Varicella vaccination or written documentation that your child has had chickenpox.

An eye examination performed by an ophthalmologist or optometrist before the age of five. Examinations by a pediatrician or family care physician do not meet the standard.

A physical examination with age appropriate systems assessment and anticipatory guidance.

Immunization exemptions will only be accepted for students with an exemption on file before January 2016.
 
Required for all students entering 6th grade:

An updated immunization certificate with documentation of a Measles, Mumps, and Rubella (MMR) booster, Tetanus (Td) booster, a meningococcal immunization, and a completed 3-step Hepatitis B series.

An updated physical examination with age appropriate examination and anticipatory guidance.

Immunization exemptions will only be accepted for students with an exemption on file before January 2016.
 
Annual Requirement for Sports Physicals:

A signed copy of a sports physical that clears students to participate in athletic offerings at TLS.

Sports physicals are not interchangeable with 6th grade physicals.
 
Birth Certificate
No child shall be eligible for entrance into The Lexington School without presentation of a Certificate of Live Birth (copies are fine) issued by the state or country of birth. Hospital records (generally with the name of the hospital and footprints or designs) are recognized as heirlooms but not legal documents. If the legal name on the certificate does not match the name your child currently uses (nicknames excluded), it is essential the amended birth certificate is part of the permanent record also.
 
Physical Examinations
All students new to The Lexington School shall present documentation of a physical examination within 180 days prior to initial entry or 30 days after enrollment.
